How does the "Island Lifestyle" compare to California?

It’s “California without the crowds.” You trade the PCH for the Malahat and 405 traffic for 15-minute commutes. You get the same rugged coastline and surf culture (Tofino), but with a much slower, community-focused pace of life.

Will I need to re-do my residency?

No. As of 2025, BC has expanded reciprocity. If you are ABMS or ABFM certified and trained in an ACGME-accredited program, you are likely eligible for full licensure without further exams.

What is the commute like?

Traffic is virtually non-existent compared to CA. Most doctors on the Island live within 10–20 minutes of their clinic or hospital, often with ocean or forest views along the drive.

How does the cost of living compare?

While gas is pricier, your overall “burn rate” is often lower. Housing in cities like Nanaimo or Comox is significantly more affordable than the Bay Area or LA, and you’ll save thousands on private health insurance premiums for your family.

What are the tax implications for a US citizen?

You will have dual filing obligations (IRS and CRA). However, the US-Canada Tax Treaty prevents double taxation on the same income. We recommend working with a cross-border accountant to optimize your transition.

Does the Island have a "fire season"?

BC does experience wildfires, but the Island’s maritime climate and high humidity levels often result in better air quality and fewer local risks compared to the California interior.

Can I maintain my specialty focus?

Absolutely. While we need generalists, our regional hubs (Victoria, Nanaimo, Campbell River) are looking for specialists who want to lead departments without the “big city” bureaucracy.

What are the schooling options?

BC’s public schools are world-class and free. For those preferring private education, the Island hosts prestigious “West Coast” boarding and day schools like Brentwood College and St. Michaels University School.

How easy is it to fly back to CA?

Very. Victoria (YYJ) and Nanaimo (YCD) offer quick flights to Vancouver or Seattle, with direct connections to LAX, SFO, and San Diego.
